Order 688 of April 6. I. General provisions. III. Procedure for conducting an independent quality assessment

In pursuance of Federal Law No. 128-FZ of July 25, 1998 “On State Fingerprint Registration in the Russian Federation,” we order:

1. Approve the attached Regulations on the procedure for the formation and maintenance of the information array created in the process of state fingerprint registration.

2. Consider as invalid the Order of the Ministry of Internal Affairs of Russia, the Ministry of Emergency Situations of Russia, the Ministry of Defense of Russia, the Ministry of Taxation of Russia, the Ministry of Justice of Russia, the State Customs Committee of Russia, the Foreign Intelligence Service of Russia, the Federal Military Service of Russia, the Federal Federal Service for Internal Affairs of Russia, the Federal Migration Service of Russia, the FSB of Russia, the Federal Tax Service of Russia, the Federal Security Service of Russia, the Federal Border Guard Service of Russia, FAPSI from November 17, 1999 N 643/531/549/AP-Z-24/364/331/786/82/112/363/96/423/413/357/620/189 “On approval of the Regulations on the procedure for the formation and maintenance of information array created in the process of state fingerprint registration."

3. Control over the implementation of this Order shall be entrusted to the deputy heads of federal executive authorities through the departments under their supervision.

Regulations on the procedure for the formation and maintenance of the information array created in the process of state fingerprint registration

1. These Regulations determine the general procedure for the formation and maintenance of the information array created in the process of state fingerprint registration, uniform approaches to the technology of obtaining fingerprint information, and the development of a regulatory legal framework. Article 1 of the Federal Law of July 25, 1998 N 128-FZ "On state fingerprint registration in the Russian Federation."

2.3. Establishing the identity of citizens of the Russian Federation, foreign citizens and stateless persons who are unable, due to health or age, to provide information about their identity.

3. The information array created in the process of state fingerprint registration includes fingerprint arrays that are independently formed and maintained by federal executive authorities.

4. Obtaining fingerprint information, creating and maintaining fingerprint arrays of federal executive authorities are carried out in the manner determined by the regulatory legal acts of federal executive authorities, in accordance with the requirements of the legislation of the Russian Federation. The information technology for the functioning of fingerprint arrays of federal executive authorities must be compatible with the information technology adopted by the internal affairs bodies. Article 3 of the Federal Law of July 25, 1998 N 128-FZ “On state fingerprint registration in the Russian Federation.” Clause 4 of the Regulations on sending material media containing fingerprint information to internal affairs bodies, approved by Decree of the Government of the Russian Federation of December 25, 1998 N 1543 (Collected Legislation of the Russian Federation, 1999, N 1, Art. 193; 2006, N 3, Article 297; 2008, No. 18, Article 2056; No. 48, Article 5604; 2010, No. 30, Article 4093).

5. Restrictions on access to information contained in fingerprint arrays are established by authorized officials of federal executive authorities in accordance with the legislation of the Russian Federation. Such information is handled in accordance with the requirements of the legislation of the Russian Federation in the field of information, information technology and information protection.

6. The information array created in the process of state fingerprint registration consists of the following fingerprint arrays:

6.1. An array of fingerprint information about the personnel of the Russian Ministry of Defense (except for the personnel of foreign intelligence agencies), the Russian Ministry of Internal Affairs, the federal executive body authorized to exercise control and supervision functions in the field of migration, its territorial bodies, organizations and divisions, the Federal Customs Service of Russia, the FSSP of Russia , the Federal Penitentiary Service of Russia, the Ministry of Emergency Situations of Russia, professional emergency rescue services and professional emergency rescue units, the Ministry of Transport of Russia, Rostransnadzor, Russian Aviation and their territorial bodies, as well as persons dismissed from service (work) from the federal security service, federal government agencies security, Foreign Intelligence Service of Russia, bodies for control over the circulation of narcotic drugs and psychotropic substances, the Federal Tax Service of Russia and its territorial bodies, foreign intelligence bodies of the Russian Ministry of Defense, the formation and maintenance of which is carried out by the Ministry of Internal Affairs of Russia. Internal affairs bodies ensure compliance with the processing regime and rules for providing information, which are established by the federal executive body sending material media containing fingerprint information.

6.2. An array of fingerprint information about the personnel of the federal security service and citizens entering military service under a contract, the federal state civil service or work in these bodies, the formation and maintenance of which is carried out by the FSB of Russia.

6.3. Arrays of fingerprint information about employees of foreign intelligence agencies: a fingerprint array of employees of the Russian Foreign Intelligence Service, the formation and maintenance of which is carried out by the Russian Foreign Intelligence Service; a fingerprint array of employees of foreign intelligence agencies of the Russian Ministry of Defense, the formation and maintenance of which is carried out by the Russian Ministry of Defense.

6.4. An array of fingerprint information on the personnel of federal state security agencies, the formation and maintenance of which is carried out by the FSO of Russia.

6.5. An array of fingerprint information about the personnel of the bodies for control of the circulation of narcotic drugs and psychotropic substances and an array of fingerprint information about persons suspected of, accused of committing crimes, subjected to administrative arrest and who have committed administrative offenses, the formation and maintenance of which is carried out by the Federal Drug Control Service of Russia.

In pursuance of the Federal Law of November 21, 2011 No. 323-FZ “On the fundamentals of protecting the health of citizens in the Russian Federation”, in accordance with the Territorial program of state guarantees of free provision of medical care to citizens in the Republic of Crimea for 2017, approved by the resolution of the Council of Ministers of the Republic of Crimea dated December 29, 2016 No. 658 “On the Territorial program of state guarantees of free provision of medical care to citizens in the Republic of Crimea for 2017”, by order of the Ministry of Health of the Republic of Crimea dated 04/18/2016 No. 557 (as amended and supplemented), Regulations on the Ministry of Health of the Republic of Crimea , approved by Resolution of the Council of Ministers of the Republic of Crimea dated June 27, 2014 No. 149, in order to effectively use the available resources of computer and magnetic resonance imaging scanners in medical organizations implementing the State program “Health Modernization Program of the Republic of Crimea”, as well as for the purpose of organizing the accounting and selection procedure and referrals of outpatient and inpatient patients to computed tomography (hereinafter referred to as CT) and magnetic resonance imaging (hereinafter referred to as MRI),

I ORDER:

  1. Approve:
  2. 1.1. The procedure for selecting, referring patients for CT, MRI and conducting research (Appendix No. 1 to this order).
    1.2. Form of a standard referral form for a CT scan (Appendix No. 2 to this order).
    1.3. List of indications and contraindications when referring patients for CT and MRI (Appendix No. 3 to this order).
    1.4. The procedure for accounting and registration of patients undergoing CT and MRI (Appendix No. 4 to this order).
    1.5. “Informed voluntary consent to conduct magnetic resonance imaging” (Appendix No. 5 to this order).
    1.6. “Form for recording the conducted MRI studies” (Appendix No. 6 to this order).
    1.7. “Form for waiting for planned diagnostic studies for outpatients (MRI, CT)” (Appendix No. 7 to this order).
    1.8. Guaranteed number of places (quotas) allocated for scheduled appointment of patients for MRI (Appendix No. 8 to this order).

  3. To the heads of medical organizations subordinate to the Ministry of Health of the Republic of Crimea, who refer patients for CT and MRI:
  4. 2.1. Ensure the selection and referral of outpatients in need of CT and MRI in accordance with Appendix No. 1 to this order.
    2.2. By order of the medical organization, appoint officials responsible for the validity of referring patients for CT and MRI (chairmen of the VC), taking into account existing quotas and routing. Copies of orders should be sent to the State Budgetary Institution of Healthcare of the Republic of Kazakhstan “RKB named after. N.A. Semashko" to the chief freelance specialist of the Ministry of Health of the Republic of Crimea for radiation and instrumental diagnostics, A.V. Filippov.

    Deadline: 2nd quarter of 2017

    2.3. Refer patients for CT and MRI in accordance with orders of the Ministry of Health of the Republic of Crimea of ​​the Republic of Crimea dated 04/18/2016 No. 557 “On operating modes of computer and magnetic resonance imaging scanners in the Republic of Crimea”, 09/29/2016 No. 1554 “On amendments to the order M3 of the Republic of Kazakhstan” dated 04/18/2016 No. 557 “On the operating modes of computer and magnetic resonance tomographs in the Republic of Crimea”, 10/05/2016 No. 1601 “On amendments to the order M3 of the Republic of Kazakhstan dated 04/18/2016 No. 557 “On the operating modes of computer and magnetic resonance tomographs” in the Republic of Crimea”, Appendix No. 9 of this order.
    2.4. Familiarize medical personnel of medical organizations with this order.

  5. To the heads of medical organizations subordinate to the Ministry of Health of the Republic of Crimea, performing CT and MRI:
  6. 3.1. Ensure that research is carried out within the time frame established by the current Territorial Program of State Guarantees of Free Medical Care to Citizens in the Republic of Crimea.
    3.2. Maintain records and registration of patients in accordance with the procedure for registration and registration of patients undergoing CT and MRI (Appendix No. 4 to this order).
    3.3. Ensure and monitor the availability of documentation in the medical organization in accordance with Appendices No. 6 and No. 7 to this order.
    3.4. Provide CT and MRI rooms with landline (dedicated line) and mobile communications.

6. The response to the applicant is sent to the postal address specified when filling out the form.

7. When considering an appeal, disclosure of information contained in the appeal, as well as information relating to the private life of a citizen, is not permitted without his consent. Information about applicants’ personal data is stored and processed in compliance with the requirements of Russian legislation on personal data.

1. 1.1. In subparagraphs 2.3 and 3.3 of the order, replace the words “GKUZ “Information and Analytical Center of the Department of Healthcare” in the appropriate case with the words “GBU “Research Institute of Healthcare Organization and Medical Management of the Moscow City Health Department” in the appropriate case.

1.2. In paragraph 3 of the order, the words “Yarotsky S.Yu.” replace with the words “Gritsayuku V.B.”.

1.3. Paragraph 4 of the order should be stated as follows:

"4. 1.4. Appendix 1 to the order shall be amended in accordance with the appendix to this order.

1.5. Regulations on conducting an independent assessment of the quality of service provision by medical organizations participating in the implementation of the Territorial Program of State Guarantees of Free Medical Care to Citizens in the City of Moscow

I. General provisions

1.1. This Regulation on conducting an independent assessment of the quality of service provision by medical organizations participating in the implementation of the Territorial Program of State Guarantees for free provision of medical care to citizens in the city of Moscow (hereinafter referred to as medical organizations) was developed in order to ensure uniform approaches to conducting an independent assessment of the quality of service provision by medical organizations and establishes the procedure for conducting an independent assessment of the quality of services provided by medical organizations.

1.2. Independent assessment of the quality of service provision by medical organizations is carried out in accordance with:

1.2.1. Federal Law of January 12, 1996 N 7-FZ “On Non-Profit Organizations”.

1.2.2. Federal Law of November 21, 2011 N 323-FZ “On the fundamentals of protecting the health of citizens in the Russian Federation”.

1.2.3. Federal Law of July 21, 2014 N 256-FZ “On amendments to certain legislative acts of the Russian Federation on the issues of conducting an independent assessment of the quality of service provision by organizations in the field of culture, social services, health care and education.”

1.2.14. II. Basic principles, goals and objectives of independent assessment of the quality of service provision by medical organizations

2.1. Independent assessment of the quality of service provision by medical organizations (hereinafter referred to as independent quality assessment) is a form of public control over the activities of medical organizations.

2.2. An independent quality assessment involves assessing the conditions for the provision of services by medical organizations according to the following general criteria:

- openness and accessibility of information about the organization;

- comfortable conditions for the provision of medical services and accessibility of their receipt;

- waiting time for medical services;

- friendliness, politeness, competence of employees of a medical organization;

- satisfaction with the medical services provided.

2.3. Basic principles of independent quality assessment:

2.3.1. Compliance with the legislation of the Russian Federation.

2.3.2. Openness and publicity.

2.3.3. Voluntary participation of public associations (organizations), socially oriented non-profit organizations.

2.3.4. Independence of opinions of citizens, experts, public associations.

2.3.5. Completeness of information used in independent quality assessment.

2.4. The purpose of conducting an independent quality assessment:

2.4.1. Providing citizens with information about the activities of medical organizations.

2.4.2. Improving the awareness of consumers of medical services about the activities of medical organizations.

2.4.3. Assessment of individual performance indicators of medical organizations and timely adoption of measures aimed at increasing efficiency or optimizing their activities.

2.4.4. Timely identification of negative factors affecting the quality of medical care and their elimination.

2.4.5. Improving the quality of the organization of medical care.

2.4.6. Development of dialogue and cooperation between medical organizations and various structures of civil society.

2.5. The main tasks of independent quality assessment:

2.5.1. Increasing public awareness on issues of organizing medical care.

2.5.2. Obtaining information from citizens - recipients of medical services about the practice of providing these services by medical organizations.

2.5.3. Establishing compliance of the presentation of information about the work of a medical organization on its official website on the Internet with indicators of completeness, relevance, and convenience for visitors (patients) of medical organizations and other interested citizens.

2.5.4. Development of proposals to improve the quality of work of medical organizations.

2.5.5. Improving the quality of work of medical organizations.

2.6. The subject of independent quality assessment is:

2.6.1. Compliance of the information content and technological properties of official websites of medical organizations on the Internet, assessment of the information content and accessibility for perception of information and reference materials posted in a medical organization with the requirements of the legislation of the Russian Federation.

2.6.2. Availability of the necessary infrastructure and conditions characterizing the external and internal improvement and comfort of visitors’ stay, to the extent that satisfies public expectations, interests and needs of patients (visitors) of medical organizations, including persons with disabilities.

2.6.3. Compliance with the quality of the organization of the process of receiving patients (visitors), including the procedure for making an appointment with a doctor.

2.6.4. Assessment of the overall satisfaction of patients (visitors) with the result of the provision of medical services by a medical organization.

III. Procedure for conducting an independent quality assessment

3.1. An independent quality assessment is carried out in relation to a specific medical organization no more than once a year and no less than once every three years.

3.2. An independent quality assessment is carried out in accordance with the order of the Ministry of Health of the Russian Federation dated May 14, 2015 N 240 “On approval of methodological recommendations for conducting an independent assessment of the quality of services provided by medical organizations.”

3.3. When conducting an independent quality assessment, indicators are used that characterize the general criteria for independent assessment of the quality of services provided by medical organizations in respect of which an independent assessment is carried out, established by the Ministry of Health of the Russian Federation.

IV. Participants of independent quality assessment

Participants in the independent quality assessment are:

4.1. Medical organizations. Public (trustee, supervisory) councils of medical organizations.

4.2. Public Council of the Moscow City Health Department.

4.3. Moscow City Health Department.

4.4. Socially oriented non-profit organizations in the healthcare sector of Moscow, patient, professional medical community.

4.5. Citizens are consumers of medical services.

V. Functions of participants in independent quality assessment

5.1. Medical organizations. Public (trustee, supervisory) boards of medical organizations provide:

5.1.1. Placing and updating information on the activities of a medical organization established by regulatory legal acts of the Russian Federation, including information on independent assessment of the quality of service provision (Order of the Ministry of Health of Russia dated December 30, 2014 N 956n “On the information necessary for conducting an independent assessment of the quality of service provision by medical organizations , and requirements for the content and form of providing information on the activities of medical organizations posted on the official websites of the Ministry of Health of the Russian Federation, government bodies of the constituent entities of the Russian Federation, local governments and medical organizations on the information and telecommunications network "Internet"):

- on the official website of a medical organization on the Internet information and telecommunications network and on the personal page of the official website of the Moscow Department of Health on the Internet information and telecommunications network;

- on the official website for posting information about state (municipal) institutions on the Internet information and telecommunications network www.bus.gov.ru;

- at information stands of medical organizations.

5.1.2. Posting on the official website of a medical organization on the Internet information and telecommunications network a questionnaire for an independent survey of citizens' opinions in the form of an interactive form ().

5.1.3. The technical ability for citizens to fill out questionnaires posted on the official website of a medical organization on the Internet information and telecommunications network.

5.1.4. The ability to fill out a questionnaire to assess the quality of services provided by medical organizations on paper if citizens contact a medical organization for the opportunity to fill out the said questionnaire on paper.

5.1.5. Submitting monthly forms filled out by citizens on paper to the State Public Institution "Center for Medical Inspection of the Department of Health of the City of Health".

5.1.6. Providing assistance to representatives of non-profit socially oriented public organizations (associations) in conducting an independent assessment of the quality of service provision.

5.1.7. Carrying out activities to improve the quality of work of a medical organization and access to information about the medical organization necessary for consumers of medical services.

5.2. The Public Council of the Moscow City Health Department participates:

5.2.1. In approving the list of medical organizations participating in the implementation of the Territorial Program of State Guarantees for free provision of medical care to citizens in the city of Moscow, in respect of which an independent assessment of the quality of service provision by medical organizations is being carried out this year.

5.2.2. In the processing of primary data (questionnaires for an independent survey of citizens' opinions) in the context of medical organizations with the involvement of representatives of socially oriented public associations (organizations) in accordance with the provisions of regulatory legal acts of the Ministry of Health of the Russian Federation.

5.2.3. In the analysis of the results of an independent assessment of the quality of service provision by medical organizations and the scores of medical organizations.

5.2.4. In the formation, approval and submission to the Moscow Department of Health of scores and proposals for improving the quality of work of each medical organization and access to information about medical organizations necessary for consumers of medical services.

5.2.5. In discussing the results of an analysis of public opinion, the results of an independent quality assessment, including those formed by public organizations, professional and patient communities, and other experts.

5.3. The Moscow City Health Department provides:

5.3.1. Submitting to the Public Council of the Moscow City Health Department, before December 1 of the current year, scores of medical organizations and draft proposals to improve the quality of work of medical organizations and access to information about medical organizations necessary for consumers of medical services, based on the results of an independent quality assessment.

5.3.2. Monitoring conditions for conducting an independent assessment of the quality of service provision by medical organizations.

5.3.3. Monitoring the implementation of action plans by medical organizations to improve the quality of service delivery.

5.4. The State Government Healthcare Institution of the City of Moscow "Medical Inspection Center of the Moscow City Health Department" provides technical support for conducting an independent assessment of the quality of service provision by medical organizations participating in the implementation of the Territorial Program of State Guarantees for the free provision of medical care to citizens in the city of Moscow.

5.5. The state budgetary institution "Research Institute of Healthcare Organization and Medical Management of the Moscow Health Department" provides:

5.5.1. Posting on the official website of the Moscow Department of Health on the information and telecommunications network "Internet" of information, the provision of which is mandatory in accordance with the legislation of the Russian Federation, with the requirements for its content and form of provision, approved by Order of the Ministry of Health of Russia dated December 30, 2014 N 956n "On information necessary to conduct an independent assessment of the quality of service provision by medical organizations, and the requirements for the content and form of providing information on the activities of medical organizations posted on the official websites of the Ministry of Health of the Russian Federation, government bodies of the constituent entities of the Russian Federation, local governments and medical organizations in information -telecommunication network "Internet".

5.5.2. Organizational and technical assistance to medical organizations when they post on the official website for posting information about state and municipal institutions www.bus.gov.ru information established by the Ministry of Finance of the Russian Federation (Order of the Ministry of Finance of the Russian Federation dated July 21, 2011 N 86n “On approval of the procedure provision of information by a state (municipal) institution, its placement on the official website on the Internet and maintenance of the specified website", etc.).

5.5.3. Posting a list of medical organizations participating in the implementation of the Territorial Program of State Guarantees for the free provision of medical care to citizens in the city of Moscow, in respect of which an independent assessment of the quality of service provision by medical organizations is being carried out this year, on the official website of the Moscow Department of Health in the information and telecommunications network "Internet" " in the section "Independent assessment of the quality of service provision by medical organizations."

5.5.4. Technical capabilities for citizens to access information necessary for consumers of medical services on the official website of the Moscow Department of Health on the Internet information and telecommunications network.

5.5.5. Posting a questionnaire for an independent survey of citizens' opinions in the form of an interactive form on the official website of the Department of Health on the information and telecommunications network "Internet" (Order of the Ministry of Health of Russia dated May 14, 2015 N 240 "On approval of methodological recommendations for conducting an independent assessment of the quality of services provided by medical organizations") .

5.5.6. The technical ability for citizens to fill out posted questionnaires.

Ensuring the technical ability of patients to express opinions on the quality of services provided by medical organizations participating in the implementation of the Territorial Program of State Guarantees for free provision of medical care to citizens in the city of Moscow, on the official website of the Moscow Department of Health on the Internet and on the official websites of medical organizations in the information and telecommunications network "Internet".

5.5.7. Posting information on the results of an independent assessment of the quality of services provided by medical organizations on the official website for posting information about state and municipal institutions on the Internet www.bus.gov.ru within three working days from the date of their approval by the Public Council of the Moscow Department of Health.

5.5.8. Posting information on the results of an independent assessment of the quality of services provided by medical organizations and proposals of the Public Council of the Moscow Department of Health to improve the quality of work of each medical organization that participated in the independent quality assessment on the official website of the Moscow Department of Health in the information and telecommunications network "Internet" in the section “Independent assessment of the quality of services provided by medical organizations” within three working days from the date of their approval by the Public Council of the Moscow Department of Health.

5.5.9. Maintaining up-to-date web pages of the official website of the Moscow Health Department on the Internet information and telecommunications network, created as part of an independent assessment of the quality of services provided by medical organizations and public control of the activities of the Moscow Health Department as a government body and subordinate organizations: " Public councils of the Moscow Department of Health", "Independent assessment of the quality of service provision by medical organizations", etc.

5.6. Department of Organization of Inpatient Medical Care of the Moscow Department of Health and Department of Organization of Primary Health Care of the Moscow Department of Health:

5.6.1. Submission annually, before February 1 of the current year, to the Public Council under the Ministry of Health of the Russian Federation and to the Public Council of the Moscow Health Department of information about medical organizations participating in the implementation of the Territorial Program of State Guarantees of free provision of medical care to citizens in the city of Moscow for the current year (in the form approved by the Ministry of Health of the Russian Federation), based on the list of medical organizations approved within the framework of the Territorial Program of State Guarantees of Free Medical Care to Citizens in the City of Moscow.

5.6.2. Consideration within a month of proposals received by the Public Council of the Moscow City Health Department on the results of an independent assessment of the quality of services provided by medical organizations (to improve the quality of work of medical organizations and access to information about medical organizations necessary for consumers of medical services).

5.6.3. Formation and approval of measures to improve the activities of medical organizations (improving the quality of service provision by medical organizations and the quality of access to information about the activities of medical organizations necessary for consumers of medical services) (Appendix).

5.6.4. Participation in monitoring the implementation by medical organizations of measures to improve the quality of their services.

5.7. Department of Financial Planning and Financing of the Moscow City Health Department:

5.7.1. Organizational and methodological assistance to medical organizations of the Moscow state health care system when working with the official website for posting information about state (municipal) institutions on the Internet information and telecommunications network www.bus.gov.ru.

5.7.2. Submission in the prescribed manner of information on an independent quality assessment to the information and analytical system "Monitoring the implementation of measures to increase workers' compensation" in accordance with the order of the Ministry of Labor of Russia dated May 31, 2013 N 234a "On the forms of monitoring the implementation of the Program for the gradual improvement of the wage system in state ( municipal) institutions for 2012-2018".

5.8. Public Relations Department of the Moscow City Health Department:

5.8.1. Organizational and technical support for the activities of the Public Council of the Moscow Health Department.

5.8.2. Organizational support for the participation of representatives of socially oriented non-profit organizations in independent assessment of the quality of service provision by medical organizations.

5.8.3. Updating information on the web pages “Public councils of the Moscow Department of Health”, “Independent assessment of the quality of services provided by medical organizations” of the official website of the Moscow Department of Health on the Internet (regulatory legal acts, information and methodological materials, etc.).

5.8.4. Conducting seminars with the participation of representatives of socially oriented public organizations to disseminate best practices for independent assessment of the quality of service provision by medical organizations.
